Enrollment verification will include enrolled terms and hours, expected completion date, and degrees earned. If you need your Enrollment Verification sent to a third-party recipient (i.e. insurance company, military, etc.), please use the third signer. Florida Residency Declaration for Tuition Purposes (Dependent Student- Under the age of 24)

This form is for dependent students (as classified by the US government), meaning you’re under 24 years old and trying to prove Florida Residency. You will need a parent's/legal guardian's signature along with their two proofs of documentation (i.e. Florida driver's license, vehicle registration, and/or voter's registration card, etc.) to complete this form.

Florida Residency Declaration for Tuition Purposes (Independent Student- Ages 24 or older)

Complete and sign the TSC Residency Declaration for tuition purposes for Independent students. If you're at least 24 years old and over. To be considered an independent student, you must meet one of the following criteria (i.e. 24 years old, married, a graduate or professional student, a veteran, a member of the armed forces, a ward of the court, or someone with legal dependents other than a spouse, pursuant to the US Department of Education for the purposes of federal financial eligibility). If you're claiming independence and don't meet any of the criteria to be considered independent, you'll need to provide your most recent tax return showing that you make at least $6,300 and your two proofs of residency.

Homeless Waiver Verification Form

This waiver is for any student who lacks a fixed, regular, and adequate nighttime residence, or whose primary nighttime residence is a public or private shelter designed to provide temporary residence for individuals intended to be institutionalized, or a public or private place not designed for, or ordinarily used as, regular sleeping accommodation for human beings (defined in section 725 of the McKinney‐Vento Homeless Assistance Act, Section 1009.25, F.S., or Rule 6A‐10.087, F.A.C. ).

This waiver covers tuition and all additional in-state fees, including lab, material, and supply fees, and auxiliary fees, at Tallahassee State College and applies to credit hour courses, including college prep, and clock hour PSAV courses. Students can email their completed waiver to their Admissions Navigator or to ar@tsc.fl.edu, by mail or in person to the Admissions and Records Office. Students must submit the waiver before the 5th day of classes. Students do not need to resubmit the waiver after it has been approved while continuously enrolled but are encouraged to contact their Admission Navigator each academic year to ensure the waiver is applied for the upcoming year. Students must resubmit the waiver once they have not been enrolled for 12 or more months.  

The college has established a Foster Care and Homeless Students Liaison to provide on-campus support for children and young adults who have been or remain in the foster care system and/or who are homeless, as defined in Policy 6152. The Liaison’s primary responsibility is to assist these students in resolving any problems related to the tuition and fees exemptions specifically provided to these students under Policy 6152. Non-Degree Course Request Form

Complete this form if you do not wish to seek a degree or certificate from TSC but would like to take courses. You'll be required to upload an unofficial transcript that shows you meet the prerequisites of your desired course(s) to be approved. If you're a recent TSC graduate or a former TSC student who hasn't attended any other previous colleges/universities, you'll still be required to upload your unofficial TSC transcript.

Reverse Transfer Form

Reverse transfer is the process of awarding an associate degree after a student transfers to a Florida four-year public institution and completes degree requirements. Students who have earned at least 30 credit hours towards an associate degree at Tallahassee State College may request a Reverse Transfer once they have completed all degree requirements. Students must submit an official transcript from the transfer institution(s) to the Admissions and Records office.
